PaymentPayment is due at time of service. We accept cash, check, credit and debit cards, including Visa, Mastercard, American Express and Discover. We also offer six and twelve-month interest-free financing through CareCredit (subject to approval).
AppointmentsPlease arrive a few minutes early for your appointment to ensure that you have enough time to check in and take care of any insurance or other information changes. Your promptness allows us to stay on schedule, and we appreciate your cooperation.
MedicationPlease bring any medications, or a list of your medications, to your appointment. Include vitamins and supplements that you take regularly.
InsuranceIf you have provided us with your current insurance carrier information, we will be happy to file your dental claim on your behalf. You will be responsible for paying your co- payment or deductible at the time service is completed. If we provide services that are not covered by your insurance plan, then you are responsible for paying in full at the time of your visit, or after receiving a claim denial from your insurance company. Denied claims or claims not paid in accordance with your agreement with the insurance company should be reconciled between you and your insurance company. If your insurance company has not paid within 60 days from the time we file on your behalf, then you will be responsible for paying the balance due.

Delinquent AccountsIf your account balance has not been paid within 60 days, we reserve the right to add a 2% service charge to the balance unless alternative arrangements have been made. If your account is not paid in a timely manner, your delinquent account will be handled by a collection agency and a fee equaling 30% of the balance will be added. The balance on the delinquent account must be paid in full before you can make any future appointments at our office.
